An unannounced monitoring inspection was conducted today.
There were 16 children present with five staff supervising.
The children were eating breakfast, brushing their teeth, working puzzles, singing and dancing. Four child records were reviewed on site. Three staff records were reviewed at the main office.

Based on a tour of the playground, the facility failed to ensure that resilient surface material shall be under climbing equipment.
Evidence:
1. There are 3-4 inches, of the required 6 inches, of resilient surface material under climbing equipment.
Plan of Correction: The site supervisor reported that the resilient surface material will be raked and more material will be added to meet the depth of 6 inches.
