Inspector Notes

A renewal inspection was conducted on August 10, 2022. There were 77 children present, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, emergency preparedness and nutrition. A total of 5 children?s records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards of law and violations were documented on the violation notice issued to the program.

Based on record review, documentation of subsequent background checks were not conducted every five years.

Evidence: Staff #4 central registry check expired on 1/5/2022.
Plan of Correction: I will check these more and get this completed.
Standard 8VAC20-780-160-A
Based on record review, TB documentation was not completed within the last 30 calendar days of the date of employment.

Evidence: At the time of inspection (8/10/22), Staff #3 was hired 5/31/22 and TB test was completed on 3/8/22.
Plan of Correction: Going forward I will make sure I have TB test/screening on the first day.

Based on observation, electrical outlets did not have protective covers.

Evidence: Pre-K classroom did not have protective covers on 2 outlets.
Plan of Correction: Extra outlet covers from the office were given to the classroom the same day.
Standard 8VAC20-780-450-A
Based on observation and staff interview, cots did not have linens consisting of a top cover and bottom cover or a one-piece covering when used by children other than infants.

Evidence:
1.Four (4) cots in the Preschool classroom did not have bottom sheets.
2.Staff confirmed they did not have bottom sheets for 4 cots.
Plan of Correction: Extra cot sheets from the office were given to the classroom the same day.

Based on observation, diaper ointment was not labeled with the child?s name.

Evidence: The Two?s classroom, had 2 diaper ointments that were not labeled with a child?s name.
Plan of Correction: I will speak with the teachers.
