Inspector Notes

A complaint was received in the licensing office on 8/29/2022 regarding ratios. On 8/31/2022 an on-site investigation was conducted. Interviews were conducted, records were reviewed, and observations were made. At the time of this investigation, the preponderance of evidence supported the allegation and the complaint is deemed valid. Violations were found during the inspection and can be viewed on the Violation Notice.

Based on a review of records, the facility did not maintain the required staff to child ratios.

Evidence:
1. On 8/22/2022, the infant classroom was out of ratio from approximately 9:00am-11:00am. There were 10 children and 2 staff members.
Plan of Correction: We have hired staff and will maintain ratios at all times moving forward.

Evidence:
1. On 8/22/2022, the Preschool classroom was out of ratio from approximately 9:00am-1:00pm. There were 11 children with 1 staff member.
2. On 8/24/2022, the Preschool classroom was out of ratio from approximately 11:20am-1:00pm. There were 11 children with 1 staff member.
Plan of Correction: We have hired staff and will maintain ratios at all times moving forward.
