Inspector Notes

An unannounced monitoring inspection was conducted May 17, 2024 between the hours of 9:15am and 11:35am. There were 63 children directly supervised by 12 staff. The physical plant, 5 staff records, 8 children?s records, medications, and policies were inspected. Children were observed participating in circle time, centers and playing outside. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website. REPEAT VIOLATION

Based on record review and interview, the center failed to ensure that each staff member submits documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #5 (date of hire: 05/02/2024) negative tuberculosis screening was dated 05/09/2024.
2. Staff #7 confirmed that Staff #5 did not have documentation of a negative tuberculosis screening at the time of employment .
Plan of Correction: New staff will not be allowed to start working unless they have submitted a negative TB test prior to their first day of employment.
Standard 8VAC20-780-500-A
Based on observation, hand washing procedures were not followed.
Evidence:
1. Staff #6 was observed returning Child #9 to the group without washing their hands after diapering.
Plan of Correction: Staff member will be retrained on proper handwashing procedures.
