During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 29, 2025. The on-site inspection began at 9:50am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 82 children present and 12 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 10, 2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for staff #1, who is currently working, was dated one day after their first day of employment.
Plan of Correction: Going forward Director will ensure Sworn Statement is signed on employees first day of employment.

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #1, who had been employed for eight weeks, central registry search was requested four weeks after the date of employment.
Plan of Correction: Going forward employee will not be allowed to start if the central registry site is not working.

Repeat Violation

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #1, employed for eight weeks, did not have documentation of requesting a criminal history check from one state and no documentation of requesting a sex offender registry check from two states.
Plan of Correction: We did have the criminal background check, however it was not done correctly. Will ensure going forward that the request is solely requested by the center and will utilize the VDOE site to request sex offender searches.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #1 (age 18 months) last immunization record was dated over one year ago.
Plan of Correction: Management will utilize Child file spreadsheet, which automatically tells when immunization records are due, to ensure immunizations are submitted on time. We will share the need with the parent and set a deadline for submitting, if it is not submitted care will be suspended until documentation is provided.

The center shall ensure that areas and equipment of the center, outside, are maintained in a clean, safe and operable condition.

The pink push car on the Toddler playground had a broken steering wheel exposing sharp plastic.
Plan of Correction: The broken toy was removed from the playground on 1/29/25. The daily playground checklist will be used to identify broken riding toys.

Repeat Violation

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Two spray bottles of disinfectant were unlocked in the four-year-old classroom, within reach of children.
Plan of Correction: Lock will be replaced on door in the 4 year old classroom to ensure it locks properly. Work order placed.

In areas used by preschool age or younger children, electrical outlets shall have protective covers.

Five outlets were uncovered and accessible to children in a three-year-old classroom.
Plan of Correction: All Teachers will be retrained at Teacher in Service day on the company expectations that outlets are always covered. Best practice it to check the outlets throughout the day to ensure none are missing. If they are management should be notified immediately.

The center shall implement a monthly practice evacuation drill.

There was no evacuation drill completed for the month of December 2024.
Plan of Correction: Going forward the Safety Captain and Management will work together to ensure all drill are implemented according to licensing regulations

Shelter in place procedures shall be practiced a minimum of twice per year.

The last shelter in place drill was practiced one year and six months ago.
Plan of Correction: Going forward the Safety Captain and Management will work together to ensure all drills are implemented according to licensing regulations. We will also set calendar reminders to ensure the drills happen.
