During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on February 6, 2025, and completed on February 12, 2025, in response to a complaint received by the licensing office on February 5, 2025, relating to allegations of Staffing and Supervision and Programs. The inspector reviewed 3 children?s records and 6 staff records on-site on February 6, 2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 28, 2025. A POC submitted after this date will not appear on the public website.

The center must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #2, who had been employed for four months, national criminal background check was overdue by nine days.
Plan of Correction: Going forward, when staff transfer from a sister school, if the center is under a different entity, we will ensure that all background checks and TB tests are done for
our location.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2 TB was dated seven months prior to date of employment and has been employed for four months.
Plan of Correction: Going forward, when staff transfer from a sister school, if the center is under a different entity, we will ensure that all background checks and TB tests are done for
our location.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Staff #1 has been observed using harsh tones with the children in classroom #1.
Plan of Correction: Staff member will be trained on positive behavior guidance and
developmentally appropriate practices. They will be required to complete the following trainings:
