During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on March 19, 2025, and completed on April 7, 2025, in response to a complaint received by the licensing office on March 13, 2025, relating to allegations of Behavioral guidance, Forbidden actions and Supervision of children. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 14, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Child #1 (age: 4 years) and child #2 (age: 4 years) were wrestling on the classroom floor Staff #1 picked up child #1 from behind by gripping the outside of both upper arms and moved child #1 across the classroom. Child # 1 was observed having red marks on their upper right arm.
Plan of Correction: All teachers will be trained on our positive guidance and redirection policy and on how to help children in a safe way there is fight/conflict.

Repeat Violation

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Child #1 (age: 4 years) and child #2 (age: 4 years) were wrestling on the classroom floor. Staff #2 was observed calling out from across the classroom but did not move towards child #1 and child #2. Staff #1 picked up child #1 from behind by gripping the outside of both upper arms and moved child #1 across the classroom.
Plan of Correction: All teachers will be trained on our positive guidance and redirection policy and on how to help children in a safe way there is a fight/conflict.

The following actions thereof are forbidden: roughly handling a child.

Staff #1 picked up child #1 (age: 4 years) from behind by gripping the outside of both upper arms and moved child #1 across the classroom. Child # 1 presented with red marks on their upper right arm.
Plan of Correction: All teachers were trained on our positive guidance and redirection policy and on how to help children in a safe way when there is fight/conflict.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Staff #1 stated that they did not complete an injury report after they saw a bruise on child #1s (age: 4 years) upper right arm. There was no written record of this injury available for review.
Plan of Correction: All teachers will be retrained on our incident reporting procedures.
