During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 20, 2025. The on-site inspection began at 9:45am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 68 children present and 11 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 2, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-B
If the child has had a physical examination prior to attendance, it shall be within 12 months prior to attendance for children two years of age through five years of age.

The physical examination for child #1 (age: 3 years), enrolled for eleven months, was dated two years and 10 months ago.
Plan of Correction: Going forward, when a child transfers from one location to another, management will ensure an updated physical is completed upon transfer.

Over-the-counter skin products shall not be kept or used beyond the expiration date of the product.

In classroom #1, four expired diaper ointments were observed under the changing table.
Plan of Correction: The expired ointments were discarded immediately. All staff have been re-trained on the expectations for over-the-counter skin products. Management will check ointments on a monthly basis to ensure they are not expired.

If diaper ointment or cream is used, these products do not need to be kept locked but shall be inaccessible to children.

In classroom #1, diaper ointment was observed in an unlocked cabinet under the changing table, accessible to children.
Plan of Correction: The cabinet was immediately locked in order to make the diaper ointment inaccessible to the children. All staff have been re-trained on the safe storage of
topical ointments and creams as part of daily diapering procedures and spot checks will happen weekly by management to ensure compliance.

Any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children.

The Virginia State Registration stickers on the bus used to transport children, were expired and overdue by over two months.
Plan of Correction: The vehicle will not be used until the registration is made current. Going forward a calendar reminder will be used to ensure the vehicle registration is completed on time.

Repeat Violation

Each center shall maintain and keep at the center a separate record for each child enrolled.

At the time of inspection, there was no record available to review for child #2.

compliance.
