During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on August 20, 2025, and completed on September 5, 2025, in response to a complaint received by the licensing office on August 18, 2025, relating to allegations of Staffing and supervision and Programs. The Department issued a Notice of Intent to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on August 5, 2025, as a result of high-risk violations. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 12, 2025. A POC submitted after this date will not appear on the public website.
Violations
3When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff #1 was observed by Witness #1 and #2 spraying multiple children in the face with a spray bottle of water. Witness #1 stated that children would be sprayed in the face with water by Staff #1 if they were talking to each other or not lining up to go outside. Witness #2 stated children would be sprayed in the face with water by Staff #1 to get in line to go outside and stay in line.
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Witness #1 and #2 observed Staff#1 use a bottle of water to spray multiple children in the face in classroom#1. Witness #1 stated children `s faces were sprayed by Staff #1 when they would not line up to go outside or talked to each other. Witness #2 stated children?s faces would be sprayed by Staff #1 to get in line to go outside and stay in line.
The following actions or threats of physical punishment and roughly handling are forbidden.
According to Witness #1 and #2, Staff#1 sprayed multiple children in the face with water when the children wouldn?t line up and to stay in line on their way outside to the playground.
