During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on September 17, 2025, and completed on October 1, 2025, in response to a complaint received by the licensing office on September 15, 2025, relating to allegations of Special Services. The Department issued a Special Order to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on August 26, 2025, as a result of high-risk violations. The inspector reviewed 4 children?s records and 0 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Special Services and Administration and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 8, 2025. A POC submitted after this date will not appear on the public website.

Records and reports on children required by this chapter shall be maintained and made accessible for two years after termination of services.

There was no record of a feeding schedule for formula for Child #1 (age 9 months) who unenrolled one month ago.
Plan of Correction: Going forward, all infant feeding schedules will be copied and filed for at least two years following the child?s withdrawal from the center. Administrative staff will be responsible for maintaining these records and ensuring they are filed appropriately.

The record of each child on formula shall contain the child's feeding schedule.

The record for Child #2 (age 11 months) did not contain a feeding schedule. Child #2 drinks formula.
Plan of Correction: Going forward, all children aged 15 months and under will have an individual, signed feeding schedule on file, including those following the center?s standard feeding times. Staff will ensure that this form is completed and signed by the parent or guardian at the time of enrollment. Documentation will also be reviewed and monitored by administrative staff.
