Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/19/2021 and concluded on 4/22/2021. The Director was contacted by telephone to initiate the inspection. There were 72 children present with 13 staff. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed 4 children?s records, 4 staff records and 1 medication and authorization submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.
Violations
1Evidence:
1. Staff #2's (date of hire: 2/24/2021) sworn disclosure statement confirms that they have resided in Maryland within the last 5 years.
2. The program Director stated that a Maryland central registry background check had been filled out for Staff #2 but the results have not been received or followed up with.
3. The program Director confirmed that a Maryland sex offender registry search has not been completed for Staff #2.
