Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 9:20am and 11:20am. There were 7 classrooms observed with a total of 64 children with 14 staff within the supervision guidelines. The children were observed playing outside, participating in circle time, playing board games and other organized activities. Diaper changing procedures were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review, the center has not obtained documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #1 (13 months) most recent documented immunizations are from when the child was 2 months old.

Based on observation, areas of the center were not maintained in a clean, safe and operable condition.
Evidence:
1. There is a large area of peeling paint under the window in the Beginner classroom.
2. There are small areas of peeling paint in the reading nook area of the Pre-K classroom.
3. There is a large area of exposed plaster near the cubby storage in the Pre-K classroom.
4. The laminate flooring is lifting up around the drain in the Pre-K and Pre-K2 classrooms.
