Inspector Notes

A complaint was received in the licensing office on 9/5/2023 regarding classroom ratios and supervision. On 9/12/2023 an on-site investigation was conducted. Interviews were conducted and observations were made. At the time of this investigation, the preponderance of evidence supports the allegation and the complaint is deemed valid. Areas of non-compliance are identified on the Violation Notice.

Standard 22.1-289.011-F
Based on observation, the most recently issued violation notice is not posted at the facility.
Evidence: The violation notice posted is dated 12/16/2022. The most recent inspection of the facility was 8/11/2023.
Plan of Correction: I printed it and must have forgotten to hang it up. This one will get posted.
Standard 8VAC20-780-350-B-1
Based on observation, the staff-to-children ratio of 1:4 for children birth-16 months was not followed when children were in care.
Evidence: At approximately 9:10amthere were 5 children observed with 1 staff in the older infant classroom. Staff confirmed that the youngest child in the classroom was approximately 15 months old.
Plan of Correction: We had one staff scheduled to be out today and a second teacher called out sick. Normally the classrooms are fine. We also have two new teachers scheduled to start on Monday, 9/18/2023.

Based on observation, the staff-to-children ratio of 1:5 for children 16 months-24 months was not followed when children were in care.
Evidence: At approximately 9:12am, there were 6 children observed with 1 staff in the toddler classroom.
Plan of Correction: We had one staff scheduled to be out today and a second teacher called out sick. Normally the classrooms are fine. We also have two new teachers scheduled to start on Monday, 9/18/2023.

Based on observation, the staff-to-children ratio of 1:8 for children 2 years old was not followed when children were in care.
Evidence: From approximately 8:50am-9:00am, there were 14 children observed with 1 staff.
Plan of Correction: We had one staff scheduled to be out today and a second teacher called out sick. Normally the classrooms are fine. We also have two new teachers scheduled to start on Monday, 9/18/2023.

Based on observation, the staff-to-children ratio of 1:10 for children 3 years up to school age was not followed when children were in care.
Evidence:
1. At approximately 8:55am, 16 children were observed with 1 staff in the Pre-K classroom.
2. At approximately 9:00am, 19 children were observed with 1 staff in the Pre-K2 classroom.
Plan of Correction: We had one staff scheduled to be out today and a second teacher called out sick. Normally the classrooms are fine. We also have two new teachers scheduled to start on Monday, 9/18/2023.

Repeat Violation
Based on observation, a copy of a special order issued by the department was not posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence: A special order was issued to the licensee on 6/8/2023. The special order was not posted anywhere in the facility.
Plan of Correction: I had to take it down because it was getting dirty from the cleaners spraying the doors every day. I'll print a new copy and get it posted.
