Inspector Notes
A complaint was received in the licensing office on 10/4/2023 regarding classroom ratios and supervision. On 10/13/2023 an on-site investigation was conducted. Interviews were conducted and observations were made. At the time of this investigation, the preponderance of evidence supports the allegation and the complaint is deemed valid. During the investigation, additional non-complaint related violations were found. Areas of non-compliance are identified on the Violation Notice.

Violations
7Based on observation, areas and equipment of the center were not maintained in a clean and safe condition.
Evidence:
1. The soft blue childrens couch in toddler classroom had several tears and rips along the seams.
2. A wall in the pre-k classroom next to where the cots are stacked has large holes, peeling paint and the plaster is exposed.
Evidence: The staff person observed leading the toddler classroom does not meet the qualifications of a program leader.
Based on observation, the staff-to-children ratio of 1:5 for children 16 months-24 months was not followed when children were in care.
Evidence: At approximately 9:10am, there were 6 children observed with 1 staff in the toddler classroom.
Based on observation, the staff-to-children ratio of 1:10 for children 3 years up to school age was not followed when children were in care.
Evidence: At approximately 8:55am, 21 children were observed with 2 staff in the Pre-K classroom.
Evidence:
1. Center management confirmed that the individual staff member observed in the toddler classroom does not have current CPR certification.
2. Center management confirmed that the two staff members observed in the pre-k2 classroom do not have current CPR certification.
Evidence:
1. Center management confirmed that the individual staff member observed in the toddler classroom does not have current first aid certification.
2. Center management confirmed that the two staff members observed in the pre-k2 classroom do not have current first aid certification.
Evidence: On 10/4/2023, child #1 tripped in the pre-k classroom and needed to receive stitches in their lip. Center management was not able to produce the injury report when requested on 10/13/2023.
