Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 11:45am and 1:30pm. There were 6 classrooms observed with a total of 65 children with 10 staff within the supervision guidelines. The children were observed eating lunch (fish sticks, mixed vegetables, pineapple and milk) and transitioning to naptime. A complete inspection of the physical plant, children and staff records and emergency procedures were observed during this inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

1. Staff #1s most recent sworn disclosure statement is dated 11/9/2018.
2. Staff #2s most recent central registry background check is dated 11/7/2018.
Evidence: Staff #3s record does not contain documentation of a sworn disclosure statement. Center management confirmed that a sworn disclosure statement was not completed.
Evidence: Staff #2 (date of hire: 1/8/2024) submitted a tuberculosis screening dated 5/11/2022 upon hire.
Evidence: Staff #4 (date of hire: 9/18/2023) and staff #5 (date of hire: 8/28/2023) have not completed the VA Dept. of Education Preservice training.
Based on observation, the center did not maintain a current written list of children's allergies in each classroom where children are present.
Evidence: The centers most recently updated allergy list is dated November 2023. The allergy list in the intermediate classroom is dated July 2023.
Evidence: Children in the intermediate classroom were observed asking for second helpings of fish sticks during lunch. Staff in the classroom stated that there were not additional helpings of food available.
