Inspector Notes

An unannounced inspection was initiated and completed on 8/2/2024 in response to a complaint
received by the licensing office relating to allegations of documentation of injuries. The
inspector reviewed 1 children?s records during the investigation. This inspection included document review (i.e. Injury logs, policies and procedures) and interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

REPEAT VIOLATION
Based on record review and interview, written records of children's serious or minor injuries did not include all required information.
Evidence: An injury report for child #1 did not contain documentation of treatment provided, future actions to prevent recurrence or two signatures (staff member and parent OR two staff members).
Plan of Correction: This situation was handled differently because there were not visual marks of an injury. Moving forward, we will make sure the injury reports are filled out entirely.
