Inspector Notes

An unannounced inspection was initiated on 7/24/2025 and completed on 7/30/2025 with a local agency, in response to a complaint received by the licensing office on 7/24/2025 relating to allegations of abuse. The inspector reviewed 1 child records and 2 staff records on-site on 7/24/2025. This inspection included document review and interviews

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to reporting abuse and neglect and are documented on the violation notice issued to the program.

Standard 63.2-1509-A
Any staff who, in their professional capacity, has reason to suspect that a child is abused or neglected, shall report the matter immediately to the local department of the county wherein the abuse or neglect is believed to have occurred.

Staff #1 stated that they observed staff #2 yank and punch child #1 in the chest. Staff #1 stated that they observed the alleged abuse in April 2025 but did not report the incident to the program director until the end of May or possibly beginning of June. Staff #1 never reported the alleged abuse to the local department for investigation.
