Inspector Notes

A monitoring inspection was initiated on March 14, 2023 and concluded on March 15, 2023. There were twelve children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, facility did not ensure hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: Hand sanitizer was stored on a cabinet approximately 3' tall and on top of file cabinet approximately 5' tall. The hand sanitizers were labeled "Warnings and Keep out of Reach of Children".

Based on review, facility did not ensure 5 of 5 children's records have documentation of required information.

Evidence: The following was found during review of children's records:
1. The records for child #1, #2, #3, #4 and #5 did not document start date of attendance.
2. The record for child #3 did not document emergency contact addresses.
