Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/19/2025 and completed on 6/25/2025. The on-site inspection began at 2:35 pm and ended at 3:25 pm. The inspector reviewed compliance in the areas listed above.There were 4 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records electronically on 5/21/2025. This inspection included document review, tour of the facility, interviews, observations and measurement.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, must be maintained in a clean, safe and operable condition. There was a broken sliding cabinet door under the hand washing sink used by children. The door was off of its track and had water damage that caused the wood to splinter.
Plan of Correction: Staff #1 stated that the door had broken the same morning of the inspection. A maintenance request was completed and the repair should be done no later than by the end of the week. Staff will ensure that children do not have access this cabinet door until it is repaired.
