Inspector Notes

A monitoring inspection was initiated on 1/10/23 and concluded on 1/10/23. There were 71 children present, ranging in ages from 2 to 5, with 14 teachers and 2 administrators present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and three staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-130-A
Based on record review and interview, the center failed to obtain the immunization record for two out of four staff records reviewed prior to the children's first date of attendance.

Evidence:

1. Child 1's first date of attendance was documented as 1/3/23. The record did not contain an immunization record.
2. Child 1's first date of attendance was documented as 1/4/23. The record did not contain an immunization record.
3. The director verified both children are attending the center and they do not have immunization records for either child at this time.
Plan of Correction: Both parents are working on getting the immunization records. In the future children will not be allowed to start until the immunization records are obtained.
Standard 8VAC20-780-60-A
Based on record review and interview, the center failed to ensure one out of four children's records reviewed contained all the required information.

Evidence:

1. The record for child 1 did not contain the father's employer and phone number.
2. The director stated she was unaware if the father of child 1 worked or not.
Plan of Correction: The parents of child 1 will be required to provide the work information for the child to be added to the record. In the future paperwork will be reviewed prior to the first day of attending to ensure all required information is provided.
