Inspector Notes

A monitoring inspection was initiated on 02/21/2024 and concluded on 02/21/2024 from 10:00 AM to 12:15 PM. There were 73 children present, ranging in ages from two to five, with 13 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and five staff?s records and board officer?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure agents of the application for licensure updated central registry background check, fingerprint background check and sworn statement every five years.

Evidence:

1. The fingerprint results for board officer 1 are dated 9/17/18, the central registry results are dated 7/26/17 and the sworn statement is dated 7/11/17.
2. The director stated the background checks for board officer 1 have not been updated.
Plan of Correction: Board officer 1 will come in the complete all required paperwork. Once the results are received they will be forwarded to the licensing inspector.

Based on record review and interview, the center failed to have an annual approval from the health department.

Evidence:

1. The most current health inspection is dated 2/6/2023.
2. The director verified there is not a more current health inspection.
Plan of Correction: A health inspection was scheduled the day of the inspection for 2/23/2024. The results will be sent to the licensing inspector upon receipt.
