An unannounced monitoring inspection was initiated on 03/31/2022 and concluded on 04/14/2022 to investigate a self-reported incident received by the department involving the area of supervision. The director was contacted by telephone to conduct the investigation.
The inspector conducted interviews with staff, and documents and video footage were reviewed.
