Inspector Notes

A monitoring inspection was conducted on 03/27/2023 between the hours of approximately 11:20 am and 2:30 pm. There were 40 children present, with 9 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Rooms were measured as requested by the center's director.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on record review, the center did not ensure that documentation of negative tuberculosis (Tb) screening for each employee is submitted at the time of employment and prior to coming into contact with children.
Evidence:
1) The Tb screenings in the records for Staff #1 through Staff #4 were dated as follows:
Staff #1 (start date 03/15/2023 / Tb screening 03/23/2023),
Staff #2 (start date 12/27/2022 / Tb screening 01/17/2023),
Staff #3 (start date 12/05/2022 / Tb screening 01/04/2023),
Staff #4 (start date 12//05/2022/ Tb screening 01/05/2023).
Plan of Correction: I will ensure I am properly documenting start date and hire date to ensure all Tb is done before start date.
Standard 8VAC20-780-240-A
Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1) On the date of inspection, the record for Staff #3 (start date 12/05/2022) did not contain documentation of having completed the Virginia Department of Education-sponsored orientation course.
Plan of Correction: Staff will have training completed by 04/03/23.

Based on record review, staff records did not contain all required information.
Evidence:
1) The records for Staff #2 and Staff #4 did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.
Plan of Correction: I will have employees get letters from previous experience to have this proof.
