Inspector Notes

A renewal inspection was conducted on 02/12/2024 between the hours of approximately 9:30 am and 12:42 pm. There were 39 children present, ranging in ages from 3 months to 5 years, with 8 staff supervising. Children were observed while being bottle fed, being served and eating lunch, handwashing and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-340-D
Based on interview with the Director, the center did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director be regularly present.
Evidence:
1) On the date of inspection, there were two staff in the Two's Room. Neither of these staff had the qualifications of a program leader. Currently, a lead teacher is not assigned to the Two's Room.
Plan of Correction: We have placed a lead qualified teacher in the 2's classroom. This was corrected on the date of inspection. Prevention plan: We will ensure that each classroom has a lead qualified teacher in each classroom. We will have teachers complete lead qualifying trainings.

Based on record review, staff's records did not contain all required information.
Evidence:
1) The record for Staff #2 did not contain the following: date of employment, documentation that two or more references as to character and reputation as well as competency were checked before employment; and name, address, and telephone number of a person to be notified in an emergency
Plan of Correction: Date of employment has been documented on the employee's file. References were located and placed in the employee's file. They were checked before the employee started. The emergency contact has been added to the employee's file.
