An unannounced complaint inspection was conducted on 8/10/2022 from 3:00 ? 3:30 pm to investigate an allegation received by the department related to building maintenance and ratios. Observations were conducted. The portion of the complaint related to building maintenance was determined to be Not Valid. The portion of the complaint related to ratios was determined to be Valid. One violation was issued to the facility. An exit interview was conducted on 8/19/2022. Based on observation, the facility failed to maintain the staff-to-children ratio of one staff for 18 children school age eligible up to nine years of age. Evidence: in the kindergarten room, there were 20 children observed with one staff member. The youngest child in the room was reportedly five years old.
Plan of Correction: Children were moved during the inspection to bring the group into ratio compliance.
