An unannounced complaint inspection was initiated on 8/10/23 to investigate an allegation received by the department related to toileting procedures and a forbidden action. Based on evidence gathered through observations, interviews and documentation review, the complaint was determined to be Not Valid. An exit interview was conducted on 8/18/2023.
