Infants shall be fed on demand or in accordance with parental instructions.
1. On 5/21/2025, Child #1 (Age 4 months) was not fed in accordance with parental instructions which required giving formula every three hours. Child #1 went approximately six hours between formula feedings.
2. On 11/14/2025, Child #2 (Age 8 months) was not fed in accordance with parental instructions which required giving formula every two to three hours. Child #2 was in care for over seven hours on 11/14/2025 and was not given any formula.

Plan of Correction: The infant schedule board has been reinstalled in a visible area of the classroom for staff reference. All infant staff have been retrained on infant feeding schedules, daily routines, and the importance of following individual care plans. Additionally, all infant care plans have been reviewed and updated to ensure accuracy and consistency with parent instructions and signature.
