Inspector Notes
Today an unannounced Monitoring Inspection took place between the hours of 10am and 11:30am. There were 2 rooms observed within the supervision guidelines. The children were observed playing sports in the gym, working on art projects in the classroom, and a small group was observed leaving on a fieldtrip. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
5Evidence: The program director confirmed that none of the staff have completed daily health observation training. This was previously cited on 12/13/2021 and 3/28/2022.
Evidence:
1. At approximately 10:46am, two children between the ages of 7 and 9 years of age (based on attendance records) were observed leaving the gym unsupervised to fill water bottles and go to the bathroom down the hall.
2. At approximately 11:10am, two children between the age of 4 and 6 years of age (based on attendance records) were observed leaving the classroom unsupervised to look at the children playing outside.
3. The bathrooms are shared with the public and there are no systems to assure that individuals who are not staff members or persons allowed to pick up a child in care do not enter the public restroom area while in use by children.
Evidence: The licensee is currently licensed to enroll children ages 5 years through 12 years of age. Attendance records indicate that Child #6 was 4 years old when they enrolled in the program.
Evidence: Child #3's record does not contain a street address for their first emergency contact.
Evidence: Two reference checks have not been completed for Staff #3, Staff #4 or Staff #5.
