Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 3:00pm and 5:00pm. There were 3 rooms observed with a total of 29 children with 6 staff within the supervision guidelines. The children were observed arriving to the center from the after school buses, eating snack, playing basketball in the gym, using hula hoops in the racquetball court, using their personal computers to play games, watching a movie and being picked up by their guardians. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. The center has ample staff trained to administer medication. There were 3 medications and authorizations reviewed. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review and interview, each staff member did not submit documentation of a negative tuberculosis (TB) screening that was dated within 30 days prior to employment.
Evidence: Staff #7 (date of hire: 11/14/2022) TB screening is dated 6/13/2022.
Plan of Correction: The staff person has been terminated.
