Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code (Annual/Monitor) Inspection on September 21, 2023. There were 31 children ages 3 to 5 years old present under the direct care of 8 staff. The Licensing Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on discussion with the Center Director, the Center failed to install a carbon monoxide detector in each building built before 2015 and houses a child day program serving preschool-age children.

EVIDENCE:
Center did not have a Carbon Monoxide detector.
Plan of Correction: Center Director will purchase and install carbon monoxide detector.
