Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/02/2024 and completed on 08/23/2024. The on-site inspection began at 11:30 am and ended at 12:05 pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 4 staff records electronically on 08/23/2024. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/10/2024. A POC submitted after this date will not appear on the public website.

Based on record review and staff interview, the center did not ensure to obtain a copy of the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #2 lived outside in Virginia in the past 5 years . Information was provided on the Sworn Statement.

Staff #2 lived in Georgia. The center was not able to provide a results for the Sex Offender Check for the state Georgia and Child Abuse and Neglect Registry for State of Georgia.

Confirmed with staff.
Plan of Correction: SMSS submitted to Georgia CPS Background Screening request on 9/24/2024 . The delay was related to the correct agency in providing the screening website source. Results are pending and will be forwarded to VDOE upon reciept additionally , copies of the Georgia sex offender registry were verified.
