Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A renewal inspection was initiated on 09/14/2021 and concluded on 09/22/2021. The director was contacted by telephone and a virtual inspection was conducted. There were13 children present, ranging in ages from five to eight years , with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two children's records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews, the licensee failed to ensure that there was annual approval from the health department, as required by the Standards.

Evidence:
1. The last "Report of Environmental Sanitation Inspection" form was completed by a health department official on 10/15/2019. This form is the approval required by this Standard to show approval of the water supply, sewage disposal system, and food service.
Plan of Correction: The administration will request that the health department official complete the form annually.
