Inspector Notes
The Licensing Inspector initiated and completed an unannounced monitoring inspection on 3/22/2023. The inspector was on site from 2:30pm until 3:40pm.
There were 14 children under the care of 2 staff. The ages of the children ranged from five to ten years old. The children were observed during indoor free play and snack time. Three children?s record and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
4Evidence: Staff #2, has an employment date of 03/20/2023. A returned search of the central registry was not in the record. When requested administration could not provide the form or documentation it had been sent off by 3/30/2023.
Evidence: Staff #2, has a employment date of 3/20/2023 had no Tb screening or test on record during the inspection on 3/22/2023.
Evidence: Child #2, had an enrollment date of 8/15/2022 had no proof of ID on file during the inspection on 03/22/2023.
Evidence: Center had no emergency information on site for staff #1 or staff #2. All staff files were kept at a Central location offsite.
