Inspector Notes
An unannounced complaint inspection was initiated on 07/19/2023. The inspector was on site from 2:00pm until 2:45pm.
Interviews were conducted with staff and three staff records were reviewed on a later date.
Information gathered during the inspection determined the complaint to be valid. Non-compliance with applicable standards or law, and violations complaint and non-complaint related are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain a completed Sworn Statement prior to first day of employment for staff member.
Evidence: Staff # 3 with an unknown hire date, had no documentation of a sworn statement in the record during the inspection. Staff #3 did not provide documentation when requested.

Based on record review and staff interviews, the center failed to ensure that staff shall complete orientation training prior to the staff member working alone with children and no

Based on record review and staff interviews, the center failed to ensure that there shall be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

REPEAT VIOLATION
Based on the observation, the center failed to ensure during the hours of operation, there always shall be on the premises one staff who meets the qualifications of program leader and an immediately available staff member, volunteer or other employee wo is at least 16 years of age, with direct means for communication between the two of them if there is an emergency.
Evidence: Staff #1 stated that they open the building in the morning sometimes by themselves. Staff #1 stated them being alone just depends on if they have had call in or not. Staff #1 stated sometimes there is only one person in the afternoon once numbers get down.
