Inspector Notes

An unannounced mandated inspection was conducted on 05/16/2022. There were 33 children, ages 5-12 years and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition.

A total of 5 children?s records and 3 staff records were reviewed. The children were observed during arrival to the program and during snack time. The inspector arrived for the inspection at 2:15 pm and departed the center at 4:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
