Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 5/29/2025 and completed on 5/29/2025, as a part of the licensure period. The on-site inspection began at 3:29PM and ended at 4:15PM. The inspector reviewed compliance in the areas listed above. There were 33 children present and 2 staff. The inspector reviewed 5 children?s records on site on 5/29/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, Child #2, Child #3, Child #4 and Child #5 ,enrolled for 8 months, were missing 1 required component. The record for Child #1, Child #2 ,enrolled for 8 months, were missing 2 required components.

Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #2, who has been enrolled for 8 months, has a diagnosed food allergy and did not have a written care plan in their file.
