During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on March 26, 2025. The on-site inspection began at 1pm and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. There were 91 children present and 18 staff. The inspector reviewed 11 children?s records and 8 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-A
All employees shall undergo a background check and every five years thereafter. Staff #2?s criminal background check expired on January 2, 2024.
Plan of Correction: The center responded with the following: Staff #2 will schedule an appointment with fieldprint and obtain up-to-date background checks. Moving forward, all employees will have their background checks completed every five years.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1 (employed for 4 months) and Staff #3 (employed for 2 years) do not have a completed national criminal background check.
Plan of Correction: The center responded with the following: The RECDC will obtain the criminal background checks for both Staff #1 and Staff #3 and provide a copy to the Department of Education. Moving forward, all criminal background checks will be completed prior to the employee?s first day of employment.
Standard 22.1-289.035-B-4
Any individual required to undergo a background check shall authorize the child day center to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry in which the individual has resided in the preceding five years. Staff #4 disclosed on her sworn statement that she lived in another state outside of Virginia the past 5 years and no out-of-state background checks were completed.
Plan of Correction: The center responded with the following: The center will obtain the out-of-state background checks of Staff #4 and place the results in her record as well as send a copy to the Department of Education.
