Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, April 19, 2022 from 11:10am to approximately 1:15pm. There were a total of 34 children in care in the direct care of 6 staff members. The center?s director was present and assisted the inspector throughout the inspection. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, hallways, bathrooms, and outdoor play areas were inspected. The required postings were reviewed and found to be in compliance. Medication is administered when required and medication and medication authorizations were reviewed. During the inspection, five children?s records and six staff records were reviewed. Additional documents were submitted electronically and reviewed virtually.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: 1) The record for Staff #2, hired on 08/27/21, had fingerprint results that were dated 08/30/21. 2) During interview, a member of management confirmed the fingerprint results for Staff #2 were received after their hire date.
Evidence: 1) The record for Staff #2, hired on 08/27/21, indicated the staff had resided in another state outside of Virginia within the last five years. The record did not contain a criminal history record information check, a sex offender registry check, or a search of the child abuse and neglect registry. The out-of-state criminal history record information check and the sex offender registry check are required to be obtained prior to hire. The out-of-state search for founded complaints of child abuse or neglect is required to be requested within the first 30 days of being hired. 2) During interview, a member of management confirmed the required out-of-state checks were not obtained for Staff #2 within the required time frames.
Evidence: 1) The TB screening in the record for Staff #1, hired on 04/13/22, was dated 02/28/22. 2) During interviews, a member of management confirmed the TB screening in the record for Staff #1 was completed more than 30 calendar days of the date of employment.
Evidence: 1) A medication authorization, for a prescription medication, was observed for Child #6. The center did not have written authorization from the child?s physician and parent for a long-term medication. The center had written authorization from the child's parent to administer the medication, but the written authorization was not renewed after 10 work days. The medication was still on site at the time of inspection.
2) During interview, a member of management acknowledged the parents were not notified that the authorization expired and that the medication had not been properly disposed of. When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
