An unannounced monitoring inspection was conducted on December 4, 2023. This on-site visit was conducted from 12:40 p.m. through 1:30 p.m. Thirty-two children, ages 8 months - 5, were in the care of nine staff. Five children and five staff records were reviewed.

Based on record review the center failed to conduct repeat fingerprint checks for three staff.
Evidence:
1. Five staff records were reviewed.
2. Staff #1's most recent fingerprint check was documented 05/10/2018. Staff #2's most recent fingerprint check was documented 09/07/2018. Staff #3's most recent fingerprint check was documented 08/31/201/.
3. The director confirmed repeated fingerprint checks had not been obtained for Staff #1. #2. and #3.
