During the inspection, the inspector reviewed the areas listed. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 09/23/2024 and completed on 11/13/2024 in response to a complaint received by the licensing office on 09/20/2024 relating to Care of Children. The inspector reviewed eight children?s records on-site. This inspection included review of documentation and interviews

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/21/2024. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
SYSTEMIC VIOLATION
Each child?s record shall contain the required information.
Child #1?s record was missing the school the child attends.
Child #2?s record was missing an address for one designated person to be contacted in case of an emergency and the parents cannot be reached.
Child #3?s record was missing the office address of the child?s physician and documentation of an annual review of the child?s emergency contact information. The only telephone number of the only parent listed had been disconnected.
Child #5?s record was missing the telephone number for the parent?s place of employment and documentation of an annual review of the child?s emergency contact information. The only telephone number of the only parent listed had been disconnected.
Child #6?s record was missing the work address for the guardian, the office address of the child?s physician, an address for one of the designated persons to be contacted in case of an emergency and the parents cannot be reached, and documentation of an annual review of the child?s emergency contact information. The last review being over two years ago.
Child #7?s record was missing a second designated person to be contacted in case of an emergency and the parents cannot be reached, the office address of the child?s physician and documentation of an annual review of the child?s emergency contact information. The only telephone numbers list for the only parent were not in service.
Child #8?s record was missing the school the child attends and the complete telephone number for one parent?s place of employment. The home phone number listed for both parents was the wrong number.

Physical punishment, including striking a child, shall be forbidden.
The caregiver struck Child #1 (age 9) with a flyswatter on 09/19/2024 resulting in Child #1 crying.

COMPLAINT RELATED VIOLATION
Verbal remarks that are demeaning to the child are forbidden.
On 09/19/2024, the caregiver told Child #1 (age 9) that their hair was dirty, they had an ugly face and the kids will make fun of them resulting in Child #1 crying.
