Inspector Notes

An unannounced, on- site monitoring inspection was initiated on July 22, 2025 and completed on July 22, 2025 as a part of the provisional licensure period. The on-site inspection began at 1:10 p.m. and ended at 1:45 p.m. The inspector reviewed compliance in the areas listed above. There were six children present and one caregiver; the point total was two. The inspector reviewed two caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises.
The findings from the most recent inspection, dated 05/14/2025, were not posted in a conspicuous place on the premises.

Standard 22.1-289.023-A
Notice of the Superintendent's intent to take any of the actions shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
The Notice of Intent dated 01/30/2025 and the Violation Notice attached to the Notice of Intent were not posted.

REPEAT VIOLATION
Potentially poisonous substances, materials and supplies such as but not limited to cleaning agents, shall be stored in areas inaccessible to children.
The laundry room, which contained numerous cleaning agents within reach of the children, was unlocked and the door was open.

A caregiver shall be physically present on site and provide direct care and supervision of each child at all times.
On July 22, 2025, Child #5 and Child #6 (ages ten and twelve years old) were dropped off at the home by another program. Caregiver #1 was not on the premises at the time and approximately fifteen minutes passed before Caregiver #1 arrived.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.
The kitchen cabinet where numerous medications were being stored and within reach of the children was unlocked. The locking device on the cabinet was broken.

When meals and snacks are provided by the family day home, the menu for the current one-week period shall be given to parents or posted or placed in an area accessible to parents.
The caregiver confirmed that the weekly menu is not given to parents. The menu for the week was not posted or placed in an area accessible to parents.
