During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on- site renewal inspection was initiated on October 22, 2025 and completed on October 22, 2025 as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Physical Equipment and Environment, Care of Children, and Nutrition.
The on-site inspection began at 3:20 p.m. and ended at 4:00 p.m. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was two. The inspector reviewed five children?s records and one caregiver/household member records on-site . This inspection included document review, tour of the facility, interviews and observation.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
