Inspector Notes
An unannounced monitoring inspection was conducted on 12/29/23 and concluded on 1/4/2024. A self-reported serious injury was also investigated during this inspection. At the time of the inspection there were 34 children in care and 6 staff members. Children were observed participating in various activities including group choice activities, relay games in the gym and art. Records were reviewed for five children and five staff members. Medication, injury reports, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Violations
4Based on a review of 5 staff records, it was determined that the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children
Evidence:
1.The records for staff #1 (date of hire 9/18/23, staff #3 (date of hire 8/17/23) and staff #5 (date of hire 7/10/23) did not contain documentation that the required TB screening had been obtained.
2.The Site Director confirmed there was no documentation of TB screenings.
Evidence:
1.There were 5 boxes or Borax cleaning solution and 5 cans of shaving cream accessible to children in an unlocked cabinet in the Enrichment Zone. The Borax and the shaving cream were labeled ?keep out of reach of children?.
2.The Director confirmed the cabinet was not locked.
Evidence:
1.While under the supervision of staff #1, child #1 slipped on a wet floor and hit his head, resulting in a concussion.
2. The injury occurred on 12/27/2023 at 3:45pm, when child #1, child #2 and child #3 were walking, skipping, and running, unaccompanied, down a hallway approximately 40 feet long.
3. It was confirmed the floor had been wet mopped approximately 15 minutes prior to the injury.
4. A yellow ?Caution Wet Floor? sign was visible at the beginning of the hallway where staff stood and did not walk down the hallway to the gym with the children.
5. After child#1 hit his head on the floor, Staff #1 was observed walking slowly to the child. Staff #1 stated the child was crying, however staff #1 was not observed providing any comfort and did not get down on the floor to assess the injured child.
6.Staff #2 was observed walking into the hallway and stood over the injured child with Staff #1 as they discussed the incident for about 2 minutes. Neither staff member got down on the floor to offer comfort to the child. Staff #2 stated child #1 was crying and screaming.
Evidence:
1.Injury reports for child #8 (report dated 11/3/23), child #9 (report date 11/15/23) and child #10 (report dated 11/22/23) did not contain the date and the parent was notified of the injury. The report for child #6 did not contain the name of the person notified of the injury.
2.The Site Director confirmed the information above was missing from the report.
