Repeat Violation
During the center's hours of operation, one adult on the premises shall be in charge of the administration of the center. Upon arrival at the center the Director was not present. The staff were not clear who was in charge during the director?s absence.

Repeat Violation
Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. The following staff, who did not meet lead teacher qualifications, were alone with three separate groups of children for approximately 30 minutes.
1. Staff #1 and Staff #2 were alone in a classroom supervising a group of 5?6-year-old children.
2. Staff #3 was alone with a group of 9-10-year-old children.
3. Staff #4 and Staff #5 were alone with a group of 7-8 year old children.

Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Children in the 9-10 year old group did not wash their hands before eating snack.
Plan of Correction: The facility responded: The staff have been retrained on proper procedures for handwashing and/or using disposable wipes before and after meal times.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiration or that a new authorization should be completed. Medication for Child #1 was onsite, but the medication authorization had expired over 30 days ago. Medication for Child #2 was onsite, but the medication authorization expired over 12 months ago. The medication had not been administered past the expiration of the authorization. The authorizations had not been renewed, and the medications were not returned or disposed of as required.
