Inspector Notes

An unannounced Code compliance inspection was conducted today from 9:15am to 11:00am. There were 55 children onsite with 8 staff providing supervision today. The children ranged in ages from2.5 years to 5 years of age. A total of 4 classrooms were occupied today. All 4
classrooms were observed along with the nap area and outside playground. Morning programming which included arts and crafts, story reading and outdoor play were observed. A total of 4 staff records and 4 children's records were reviewed. Information gathered during this inspection determined some non-compliances with applicable laws.

Standard 22.1-289.031-A-3-d
Based on observation of all classrooms, one classroom was found over ratio.
Evidence:
The Purple classroom had a total of 14 children, 3 years of age, with 1 staff. A 1 to 10 ratio was not maintained.
Plan of Correction: The assigned second staff returned after five minutes with art supplies for a morning activity. Ratios will be maintained going forward.

Based on observation of the playground, one area was not free from an injury hazard.
Evidence:
An area of the perimeter fence around the playground was missing a wood plank presenting a head entrapment hazard to children in care.
Plan of Correction: A work order will be requested to repair this area of the fence. Staff will remind children to avoid the area until repairs are complete.

Based on review of staff files, one background check was not obtained.
Evidence:
Staff #1 had residency out of the state of Virginia. An out of state criminal record check was not obtained prior to hire and has not been obtained to date.
Plan of Correction: The out of state background check will be requested asap. Results of the criminal record search will be forwarded to the Licensing Inspector within 10 days for review.
