Inspector Notes
This inspection was conducted by licensing staff including documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on September 13, 2022 and concluded on September 13, 2022. There were 56 children present, with 12 staff supervising.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) request by the 30th day of employment.
Evidence: Staff records were reviewed and 2 out of 12 did not have background checks as required, as they did not obtain the out of state CRC and the CPS check.
1. Staff #9?s first date of employment was 9-7-2022 and they had not obtained an out of state criminal records checks for Indiana and Kentucky where they had lived within the last five years.
2. Staff #12?s, first date of employment was 4-26-2022 and they had not obtained a CPS for Maryland where they had lived within the last five years.
