A n Initial subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-540-B
The center must maintain and keep at the center a record for each enrolled child that includes all of the required information and documentation. The record for child #1, who has been enrolled for 4 days, child #2, who has been enrolled for 4 days, child #3, who has been enrolled for 11 days, child #4, who has been enrolled for 11 days, and child #5, who has been enrolled for 1 month did not contain the child's living address, either parents' work address, nor the emergency contact person'saddress.
Plan of Correction: The program director states that she intends to update the enrollment forms to ensure that all of the required information is captured. These new forms will be given to parents for completion and signature and required to be returned by the end of the month. All new enrolling families will be given the updated enrollment forms from this point forward.
Standard 8VAC20-790-640-A
Hazardous substances must be in a locked place that prevents access by children. In the restroom located inside of the 4 year-old classroom, hand sanitizer and aerosol deodorizing sprays were stored under the handwashing sink in an unlocked cabinet, which was blocked by a portable set of steps.
Plan of Correction: A child lock was placed on the cabinet under the restroom sink in Pre-K 4 classroom during the inspection.
