Inspector Notes
An announced, on-site code compliance inspection was initiated and completed on 10/11/2024. The on-site inspection began at 12:35 pm and ended at 4:33 pm. The inspector reviewed compliance in the areas listed above. There were 37 children present with 7 staff. The inspector reviewed 5 children?s records and 20 staff records on site. This inspection included: document review tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-4
(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state for Staff #17 who has been employed for more than 22 months and was working in the center at the time of the inspection.
Plan of Correction: The out of state sex offender record check results for staff #17 were obtained during he inspection on 10/11/2024. All staff record files will be reviewed to ensure that any of the staff who have resided outside of the state in the past 5 years have each required component of the out of state background checks.
