Inspector Notes

An unannounced monitoring inspection was initiated and completed on 12/12/2024 in response to a self-report received by the licensing office on 10/28/2024 relating to medication administration.

The inspector reviewed 1 child record and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-770-A
The center may administer prescription medication to a child in care with written permission of the parent.
On 10/28/2024, staff #1 administered child #1's prescribed medication to child #1 in response to a medical emergency.
The center did not obtain written parent/guardian or physician authorization to administer prescription medications to Child #1.
Plan of Correction: Staff was responding to a medical emergency and was following verbal guidance from the child's parent who she was speaking to on the telephone as she administered the medication.
Written authorization has been obtained for this child's emergency medication and 5 staff have obtained the required Medication Administration Training, so far.
Standard 8VAC20-790-770-A-1
Only staff who have current Medication Administration Training (MAT) may administer prescription medication to children care.
On 10/28/2024, staff #1, who had not obtained the required MAT training, administered child #1's prescribed medication to him in response to a medical emergency.
Plan of Correction: The center did not normally administer medication or store medication on site so it did not have staff trained for medication administration. Staff was responding to a medical emergency and was following verbal guidance from the child's parent. Since this incident, five staff have obtained the required Medication Administration Training (MAT) to ensure that there is always a MAT staff on duty throughout the operating hours of the program.

Medication, except for those prescriptions designated otherwise by written physician's order, shall be kept in a locked place using a safe locking method that prevents access by children.
On 10/28/2024, child #1's prescription medication was stored unlocked in a diaper bag in the classroom at a height of approximately 6 feet from floor level.
Plan of Correction: Staff were unaware that child #1's prescription medication was located at the center. Staff were informed of the presence of the medication in his bag from home, located in the classroom, when child #1 experienced a medical emergency and the child's parent was contacted. The bag and medication were not accessible to the children in care at the time. The medication was removed from the classroom on 10/28/2024 and locked storage has been designated for any medication brought into the center.
