Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews.

A monitoring inspection was initiated on 03/9/22 and concluded on 3/14/22. The director was contacted by telephone and a virtual inspection was conducted. There were 15 children present, ranging in ages from three to five, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of documents, the facility failed to ensure that an annual health inspection is conducted.
Evidence:
1. The last health inspection on file is dated 9/9/20.
Plan of Correction: The program manager will contact the health department again to ask for an inspection.
